New patient registration (child under 16)

Use this service to register with the surgery.

• are under 16 years old or are registering a patient under 16 years old
• want to register at the surgery
• live within our catchment area

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• details of previous GP surgery
• basic health and medical information
